To all current 9th graders, (10th grade in Fall), Connecticut State Law requires a complete physical exam by MD prior to beginning 10th grade. The Blue state physical form (Health Assessment Record) must be completed and returned (or mailed) to Nurse Stanton prior to the first day of school. This Blue state form meets both sport/band and state physical exam requirements. Please note that 10th grade physical exams must be within one year of the first day of school (August 31, 2023).
 
Please note that all incoming 9th, 11, and 12th graders participating in band or sports must also have a yearly physical. Sports physicals are valid for 13 months from the date of exam and students must have a current physical throughout the entire sport season. The same Blue state physical form is used. Remember, all students participating in sports need to register online prior to try outs.
 
Any student requiring medication administration at school must have a yearly medication form submitted to Nurse Stanton for either prescription or non prescription medication, including Advil or Tylenol.
